What Is a Sliding Fee Discount Program?

The Sliding Fee Discount Program helps make healthcare services more affordable for people based on their income. Instead of charging everyone the full price, we “slide” the cost down for patients who earn less—so you pay what’s fair and manageable for your financial situation.

What Does FPG Mean?

FPG stands for Federal Poverty Guidelines. Each year, the federal government sets income levels based on household size to determine what is considered 100% of the poverty level.

For example:

  • If you make the same or less than the 100% FPG for your household size, you fall into the 0–100% income bracket.

  • If you earn more, but still under 200% of the guideline, you fall into a partial discount tier.

  • If you earn more than 200%, you pay the full price for services.

You can check your household’s FPG percentage using an income chart (we can provide one, or you can find it on hhs.gov).

How to Use This Chart

  1. Figure out your FPG bracket
    → Based on your household size and gross annual income, identify where you fall in the FPG tiers:

    • 0–100%,

    • 101–125%,

    • 126–150%,

    • 151–175%,

    • 176–200%, or

    • Above 200%

  2. Match your FPG bracket to the service you need
    → Look across the chart to find how much you’ll pay for that service.

  3. Apply through our sliding scale application
    → You may need to show proof of income (such as pay stubs or a tax return) to get approved.

  4. Receive care at a discount
    → Once approved, your discount applies to all services for the duration of your approval (typically 12 months).

Example:

Let’s say your household is at 130% of the Federal Poverty Guidelines. According to the chart:

​

  • A Therapy Session would cost you $48.00

  • A Medication Management Visit would cost you $40.00

  • A Psychiatric Evaluation would cost you $80.00

Important Notes

  • You won’t be denied care if you can’t pay full price.

  • Nominal fees (like $0–$20) are set to make sure everyone contributes something small, even at the lowest income levels.

  • You can reapply or update your status if your financial situation changes.

 Alignment with HRSA Requirements

​

  1. Full Discount or Nominal Fee for ≤100% FPGHRSA mandates that patients with incomes at or below 100% of the Federal Poverty Guidelines (FPG) receive either a full discount or a nominal charge. Your chart reflects this by offering a 100% discount with a nominal fee of $0–$20 for services like therapy sessions, medication management, psychiatric evaluations, and follow-up visits.

  2. Graduated Discounts for 101–200% FPGHRSA requires at least three discount pay classes for patients with incomes between 101% and 200% of the FPG. Your chart provides five distinct tiers within this range, offering discounts of 80%, 60%, 40%, and 20%, which meets and exceeds the minimum requirement.

  3. No Discount for >200% FPGPatients with incomes above 200% of the FPG are not eligible for discounts under HRSA guidelines. Your chart correctly applies a 0% discount for this group, indicating full payment is required.
